Subject Access Request

Data Protection Legislation gives individuals the right to request a copy of personal data that is held by the organisation. This is known as a Subject Access Request (SAR) and can be made by patients and their representatives.
 
Under the General Data Protection Regulations (GDPR), which comes into force on the 25th May 2018, SARs will be free.
 
It is vital that the organisation responds to the requester immediately and provides a copy of the record within one calendar month.
 
Please ensure all requests are sent directly to the practice address to ensure processing of the request can begin immediately.


PLEASE DO NOT ASK FOR ANY INFORMATION about patients, even relatives, as we are unable to give it. NO information will be given to any other person without the patients prior consent.

If you wish to access your medical records held at the practice then we suggest, in the first instance, that you discuss your wish with your usual doctor – they may be able to tell you the information you wish to know without you having to request a copy of your notes.

If you do not wish to discuss your request with your doctors then please contact Mrs Claire Brown (Practice Manager) who will help you further.

If your enquiry is with regard to your hospital records you will need to discuss this further with the Access to Medical Records officer at the hospital who will help you further.

If you give permission via a third party for a medical report to be completed, for whatever reason, the doctors are obliged to include all relevant information in the medical records. Under the Access to Medical Reports Act 1988 any patient is entitled to see a medical report before it is sent off. Prior notice is needed and you will be given 21 days to make any comments before the report is sent off.

Under the Data Protection Act patients are entitled to request a copy of their computer summary sheets. Please ask at reception if you require a copy.
