How To Register

All patients registering with the practice must complete a GMS1 Registration form and a new patient questionnaire. On registering with the practice all patients over the age of 5 years of age are offered a new patient medical with the practice nurse.  Please make an appointment with reception for this. 

If you wish to become a patient at Panteg Health Centre / Maendy Square Surgery please attend the surgery to complete the correct forms.   

All UK residents have the right to be registered with an NHS general practice. Panteg Health Centre and Maendy Square Surgery accept all requests to join the patient list without discrimination unless:

  • Patients are not able to prove their eligibility for NHS treatment.
  • Patients are residing out of the practice area.
  • Patients have a history of violence towards practice/NHS staff.
  • Patients have previously been removed from the practice list at the Practice Request. 

Policy on Overseas Visitors 

Not all patients who enter the UK are entitled to free NHS treatment. 

All persons registering as a permanent patient from overseas must provide the following documentation: 

Evidence of Residence                                       

  • Utility bill
  • Rent book
  • Driving licence
  • Bank statement
  • Inland Revenue/Benefits doc
  • White medical card
  • Occupancy agreement

Evidence of right to residence

  • Passport
  • Student visa
  • Work permit
  • Working holiday permit
  • ARC Card
  •  SA1 and SA2 forms
  • Ancestral visa 

Proof of eligibility as a dependent must be provided by:

  • Birth certificate
  • Marriage certificate
  • Passport 

A dependent may be classified as a wife/husband or child up to age 16 or in full time education. 

If you cannot provide the above documentation:
You will be referred to the Refugee Council in Newport. This also applies if you attend the practice stating they you are an asylum seeker.  You will not be registered onto the practice list, but can be seen as an immediately necessary patient. Visitors to the UK


As NHS contractors, GP’s have a duty to provide immediate necessary treatment (blue TR form) to any patient within their practice area, regardless of whether or not that patient is otherwise entitled to NHS care. The immediately necessary treatment means something that demands immediate treatment (i.e. chest pain breathlessness etc) and not something where the patient is just inconvenienced or forgot their medication and wants a prescription. This may be something you have to pay for.  If the patient does not require immediately necessary treatment and resides in a country that is not covered by the reciprocal agreement they must pay for a private consultation with the GP.  The amount payable is shown under the Private Fees Section on the home page.  If the patient is covered by the reciprocal agreement then they can be seen as an NHS patient with no charge.  

Temporary Patients


You are able to see a doctor anywhere in the UK if you are way from home and in need of medical help. You can do this simply by asking to see the doctor as a temporary resident and completing the necessary forms. We will be happy to see any relatives or friends staying with you if they need to see a doctor.
